Phaco Tip 149:
Foot Pedal Positioning

In order to maintain proper balance and foot pedal control of both the microscope and the phacoemulsification instrument with greatest accuracy, each knee of the surgeon should be directly (vertically) above each foot pedal. In this manner, the surgeon can best operate the pedal while remaining stable; this reduces the possibility of undesirable movement of surgical instrumentation within the eye.

This is the same principle practiced by experts in the field of martial arts. These professionals learn to maintain their weight directly over their feet so that an opponent encounters great difficulty when attempting to throw them to the ground. Surgeons who do not place the foot pedals in the correct location tend to underutilize them because they are aware that doing so causes a temporary reduction in manual dexterity.
