Tip 146: IOL Power "Errors"
Regardless of the technique (ultrasonic or optical) used
to measure axial length, refractive inaccuracy can occasionally occur.
This may result from incorrect keratometric or axial length assumption,
or a final location of the IOL that is more anterior or posterior than
predicted.
It is best to inform patients of this possibility before
surgery, as well as the treatment options that exist should this occur.
It has been my experience that doing so will usually permit the patient
to accept such an outcome with remarkable equanimity in most cases, and
this is obviously desirable.
I therefore strongly recommend that patients undergoing
IOL implantation be advised of the above prior to surgery. This can be
done in an informative, reassuring manner. I have created a document for
this purpose, and it is available on a complimentary basis to any of my
colleagues.*
