Tip 142: AC Too Deep
Rarely, the anterior chamber can be too deep for adequate
visualization during phacoemulsification. This can occur in a myopic eye
with thin, elastic sclera.
It can be dangerous to proceed with phaco when visualization
is not adequate. The simplest way to overcome this problem is to inject
a small amount (approximately .25 cc) of BSS® Sterile Intraocular
Irrigating Solution through the pars plana and into the central vitreous
cavity. A disposable 30-gauge needle is best suited for this, and will
create a self-sealing tract upon its removal.
N.B. Because the sclera can expand in these unusual eyes,
the patient will often experience discomfort when the phaco tip is inserted
and the choroid is stretched.
