Phaco Tip 132:
Burst vs. Pulse

Burst mode utilizes fixed US power applications, which are applied at variable intervals depending upon the extent of footpedal depression. Pulse mode permits variable US power to be applied, but always at a fixed interval. When to use which?

  1. Neither is helpful during sculpting, as both prolong the process.

  2. When impaling the nucleus e.g. prior to phaco chop, a very slow pulse rate (2 per sec) permits deliberate, controlled nucleus penetration.

  3. During removal of nucleus segments, the use of burst mode can greatly decrease total US energy use. Further footpedal depression within position 3 causes the frequency of the US bursts to increase, and eventually produces continuous US application. This may be necessary with very dense nuclei or, alternatively, the power level per burst can be increased in this situation.
