Capsulorhexis: Anterior Capsule Striae

During creation of the capsulorhexis, watch for striae in the anterior capsule. These folds are a sign of zonular laxity, indicating that the anterior capsule is not held tautly by the zonule. The usual precautions in such situations should be taken: lowered infusion bottle, flow and vacuum levels; cautious bimanual nuclear rotation, and utilization of adequate phaco power to avoid nucleus decentration during sculpting. In severe instances, consideration should be given to the performance of iris plane or anterior chamber phaco in these eyes.
